A middle-aged man with progressive left shoulder pain

A 52-year-old man presented with complaints of worsening left shoulder pain for the past one month. He has a history of end stage renal disease and denies any injury, trauma, chest pain or other complaints. He mentioned that the pain worsens with motion. On physical examination, the vital signs are normal. Physical examination is unremarkable except for pain on joint movement and decreased range of motion is noted in the left shoulder. A plain radiograph shows extensive amorphous lobulated periarticular calcifications.  

What is the most likely diagnosis?
